I, the undersigned, freely volunteer my services to assist the Crediton Fly Fishing Club in works associated with improving access for fishing and river habitat improvement schemes.

I have read and understood, and I agree to abide by the codes of safe practice for each activity to be undertaken.

I shall not hold the club or its members legally liable for any injury or condition sustained during or as a result of such current or future activities.
